Study Links Combined Risks of Abdominal Obesity and Vitamin D Deficiency to Increased Mortality in Older Adults
A large-scale study involving over 5,500 adults aged 50 and older has uncovered a concerning correlation between abdominal obesity and vitamin D deficiency, revealing that individuals with both conditions face a 123% higher risk of death compared to those without either issue. The findings, published in the journal Diabetes, Obesity and Metabolism, indicate that the two health risks are particularly detrimental when occurring in tandem.
Tiago Silva Alexandre, the study’s coordinator and a professor in the Department of Gerontology at the Federal University of São Carlos (UFSCar) in Brazil, explained, “Abdominal obesity is a well-known risk factor because it’s associated with inflammation and metabolic problems. Vitamin D, on the other hand, is a hormone that acts on various organs, and its deficiency impairs several bodily functions.” He emphasized the importance of monitoring vitamin D levels and addressing excess abdominal fat as critical measures for preventing premature death, particularly in older adults.
Study Overview
Conducted in collaboration with University College London (UCL) and funded by FAPESP, the research tracked 5,520 individuals for six years, all participants being part of the English Longitudinal Study of Ageing (ELSA). The study revealed that vitamin D deficiency—defined as levels below 30 nmol/L—was associated with a higher mortality risk on its own than abdominal obesity. Here, abdominal obesity was classified as a waist circumference greater than 102 centimeters for men and 88 centimeters for women.
Participants with only abdominal obesity faced a 47% increased risk of death, while those with vitamin D deficiency alone had a risk elevation of up to 91%. When both conditions were present, mortality risk rose more than twofold.
The Interaction Between Abdominal Fat and Vitamin D
Alexandre noted that abdominal obesity and vitamin D deficiency could reinforce each other in a damaging cycle. Abdominal fat can “sequester” vitamin D, storing it in fat cells and making it less available in the bloodstream. “Although the body may have the vitamin stored in fat, it isn’t freely available in the blood to perform vital functions in other organs and systems,” he explained.
Additionally, obesity can hinder the body’s ability to metabolize vitamin D. Reduced enzyme expression involved in vitamin D metabolism among individuals with obesity further limits available levels of the vitamin for bodily functions.
Aging and Inflammation
The interplay between these conditions becomes increasingly significant with age since aging is associated with inflammaging, a state of chronic low-grade inflammation. Alexandre remarked that “when vitamin D levels are low and there’s excess abdominal fat, an unfavorable systemic environment develops that accelerates cardiovascular and metabolic diseases, as well as muscle loss.”
Wider Implications of Vitamin D Deficiency
This research adds to a growing body of studies on the effects of vitamin D in aging populations. Previous work by the group has identified a cascade effect where vitamin D deficiency can lead to diminished strength and slower walking speeds, ultimately resulting in a loss of independence. They have also found links between vitamin D deficiency and cognitive decline.
Alexandre highlighted the versatility of vitamin D, noting its role in regulating various bodily functions, including blood pressure, heart rate, and immune system responses. “When its levels are low, several essential bodily functions are compromised,” he stated.
